Waivers for A Spooky Night in Nature: Camp-out at Camp Fowler

I hereby give my permission for the named registrants to participate in all activities of this program and to receive emergency medical treatment, if necessary.  I understand that the Da Vinci Discovery Center of Science and Technology, Inc., (or “Da Vinci Science Center”)  and Valley Youth House (operators of Camp Fowler) take reasonable precautions to ensure that activities are conducted by qualified personnel in a safe and responsible manner. 

I further understand that these activities may involve certain risks and exposures, which may include but are not limited to: chemicals, plants, insects, animals, and weather conditions. I recognize these risks and agree to an assumption of them by attending and/or allowing my child to attend Da Vinci Science Center and Valley Youth House programs. I hereby release, indemnify and hold harmless Da Vinci Science Center and Valley Youth House, their officers, trustees, agents, and employees from and against all losses, claims, actions, costs, expenses and/or damages, including attorney and medical fees, arising out of my participation/ my child’s participation in Da Vinci Science Center’s/Valley Youth House’s jointly run activities, their related risks, whether arising from an act or omission, negligent or otherwise, to the fullest extent permitted by law, except for the willful misconduct or gross negligence on part of the Da Vinci Science Center or Valley Youth House.

I also give permission for the above registrants’ photograph to be taken during activities, and for the organizers of Da Vinci Science Center and Valley Youth House to use the photograph image(s) in commercial or non-commercial publicity.

I agree that participants will follow normal standards of behavior.  Refusal may result in dismissal from programming.  I realize that in case of dismissal, no refunds will be issued.
